Understanding Obsessive-Compulsive Disorder (OCD)

OCD involves recurring intrusive thoughts, images, urges, or doubts and repetitive behaviors or mental acts intended to reduce distress or prevent feared outcomes. OCD is different from simply liking things organized or being perfectionistic.

  • Unwanted intrusive thoughts, images, or urges
  • Repeated checking, washing, counting, or arranging
  • Mental reviewing, repeating, or neutralizing
  • Repeated reassurance-seeking
  • Avoiding triggers or feared situations
  • Excessive doubt or need for certainty
  • Significant time spent on rituals
  • Distress or interference with daily life

How therapy may help

Exposure and Response Prevention (ERP), a form of cognitive behavioral therapy, is a well-established psychotherapy for OCD. If OCD is a primary concern, treatment should include or coordinate with a clinician trained to provide appropriate OCD-specific care.

Straight answers to common questions people ask

Does having disturbing intrusive thoughts mean I want them to happen?
Intrusive thoughts can be unwanted and inconsistent with a person's values. Their presence alone does not establish intention. A clinician can help distinguish intrusive thoughts from actual intent or risk.
Is OCD the same as perfectionism?
No. They can overlap, but OCD involves obsessions and compulsions, while perfectionism generally centers on standards, evaluation, mistakes, or self-worth.
Why does reassurance only help for a little while?
In OCD, reassurance can temporarily lower anxiety while strengthening the longer-term cycle of doubt and reassurance-seeking. OCD-focused therapy works on changing that cycle.
What treatment is commonly used for OCD?
ERP is one of the best-established psychotherapies for OCD. Treatment should be individualized, and some people also discuss medication with a psychiatric or medical provider.
